In the February 1st issue of the Journal of Infectious Diseases, scientists from the University of Washington published a study that further illustrates the potential importance of antiretroviral therapy in preventing HIV. The study of discordant couples found that one of the most important factors in predicting the likelihood of HIV transmission was viral load. As the amount of HIV in the blood increased, so did the risk of transmission during any sexual act. This explains how, and why, antiretroviral treatment could reduce a person's infectivity - the "treatment as prevention" paradigm.

The study also confirmed several other factors that are important for prevention. The scientists found that condom use was highly effective at preventing HIV. They found that genital ulcer diseases such as genital herpes increased the likelihood of transmission. They also confirmed the protective role of circumcision.

Interestingly, one unexpected result of the study was that the increased risk of transmission from men to women, as opposed to vice versa, could largely be explained by other factors - such as viral load. This flies in the face of the historical understanding that the widely observed difference was primarily due to the mechanics of exposure during sex. Men deliver a large load of potentially infected material (semen) into the vagina, where women have a large area of cells which are susceptible to infection. In contrast, women's vaginal secretions are primarily infectious to men through a much smaller surface area on the penis, and they are more easily removed after sex. It will be interesting to see if that result is borne out in further studies or if it turns out to be a fluke.

Rwanda: Mrs. Kagame Takes Part in First Ladies' Assembly Against HIV

Rwanda's First Lady, Mrs. Jeannette Kagame, was among other African First Ladies who attended the eleventh Organisation of African First Ladies Against HIV/ Aids (OAFLA) General Assembly in Addis Ababa, Ethiopia.

The meeting, whose theme is "An HIV/AIDS free generation tomorrow needs caring men and women today," is held alongside the 18th AU summit in Addis Ababa which President Paul Kagame is attending.

The First Ladies' meeting aimed at addressing issues on maternal mortality and the Elimination of Mother to Child Transmission of HIV/AIDS, revitalisation of OAFLA members and resource mobilisation.

In her speech at the opening ceremony, the President of OAFLA; the First Lady of Namibia, Mrs. Penehupifo Pohamba, underscored the need to scale up EMTCT services with special emphasis on male involvement.

A statement from the Mrs. Kagame's office indicates that, "In Rwanda the percentage of male involvement in EMTCT services is 83 percent".

"Other guests at the General Assembly included the Executive Director of UNAIDS Michel Sidibé and Mrs. Ban Soon Taek, wife of the UN Secretary General," it adds.

During the seven years since the establishment of OAFLA, the African Ladies have accomplished various goals in their countries as well as regions. The results of their efforts are increasingly visible and are making a difference in the lives of children, women and youth on the continent.

OAFLA was formed in 2002 by 37 First African First Ladies with the goal of advocating for the mobilisation of awareness and resources as well as the development of leadership, policies, strategies and actions to fight HIV/AIDS at the national, regional and global level.

Caffeine May Alter Women's Estrogen Levels

THURSDAY, Jan. 26 (HealthDay News) -- Caffeine changes women's estrogen levels and has different effects in Asian and white women, a new study says.

More than 250 women, ages 18 to 44, took part in the study between 2005 and 2007. On average, they consumed 90 milligrams of caffeine a day, about the equivalent of one cup of caffeinated coffee.

Estrogen is the reproductive hormone produced by the ovaries.

Asian women who consumed an average of 200 milligrams or more of caffeine a day (equivalent to about two cups of coffee) had elevated estrogen levels compared to women who consumed less. But white women who consumed the same amount of caffeine had slightly lower estrogen levels than women who consumed less.

Black women who consumed 200 or more milligrams of caffeine daily had elevated estrogen levels, but this finding was not statistically significant, said the U.S. National Institutes of Health researchers and their colleagues.

The caffeine consumed by the women in the study came from any of these sources: coffee, black tea, green tea and caffeinated soda. The findings differed slightly when the researchers considered the source of caffeine individually.

Consuming 200 milligrams of caffeine from coffee mirrored the overall findings. But consumption of more than one cup each day of caffeinated soda or green tea was associated with higher estrogen in all three groups of women, according to the study published online in the February issue of the American Journal of Clinical Nutrition.

The caffeine-related changes in estrogen levels did not appear to affect women's ovulation, said the researchers, who followed the women for up to two menstrual cycles.

About 89 percent of U.S. women ages 18 to 34 consume the caffeine equivalent of 1.5 to two cups of coffee a day, according to the authors.

"The results indicate that caffeine consumption among women of child-bearing age influences estrogen levels," Enrique Schisterman, of the Division of Epidemiology, Statistics and Prevention Research at the U.S. National Institute of Child Health and Human Development, said in an NIH news release.

"Short term, these variations in estrogen levels among different groups do not appear to have any pronounced effects. We know that variations in estrogen level are associated with such disorders as endometriosis, osteoporosis, and endometrial, breast and ovarian cancers. Because long-term caffeine consumption has the potential to influence estrogen levels over a long period of time, it makes sense to take caffeine consumption into account when designing studies to understand these disorders," Schisterman said.

THURSDAY, Jan. 26 (HealthDay News) -- New genetic factors associated with a woman's age when she begins menopause have been identified by an international team of researchers.

Researchers identified 13 loci (specific location of a gene on a chromosome) linked with immune function and DNA repair, which have an effect on when menopause begins, said the researchers from the Boston University Schools of Public Health and Medicine and colleagues.

They also confirmed four previously established loci.

For most women, menopause -- the term for the end of reproductive function of the ovaries -- occurs in the early 50s.

The study was published online Jan. 22 in the journal Nature Genetics.

Most previous studies examining age of onset of menopause have zeroed in on genes associated with the estrogen-production pathway or vascular components, the researchers said.

"Our findings demonstrate the role of genes which regulate DNA repair and immune function, as well as genes affecting neuroendocrine pathways of ovarian function in regulating age at menopause, indicating the process of aging is involved in both somatic and germ line aging," the study authors said.

The new findings "bring us closer to understanding the genetic basis for the timing of menopause. They may also provide clues to the genetic basis of early onset or premature menopause and reduced fertility," team co-leader Kathryn Lunetta, a professor of biostatistics at the BU School of Public Health, said in a university news release.

"We hope that as a better understanding of the biologic effects of these menopause-related variants are uncovered, we will gain new insights into the connections between menopause and cardiovascular disease, breast cancer, osteoporosis and other traits related to aging, and that this will provide avenues for prevention and treatment of these conditions," Lunetta said.

New Drug Manufacturing Plant Could Ease Shortage

A new manufacturing plant in Framingham, Mass. has received U.S. Food and Drug Administration approval to make a treatment for Fabry disease, French drug company Sanofi's Genzyme unit announced Tuesday.

The new plant, approved last week by European regulators, will help relieve a shortage of the treatment Fabrazyme. Genzyme said all U.S. patients should return to full dosing March and "unconstrained supply" worldwide will be restored throughout the year, the Associated Press reported.

The new plant was built after Genzyme experienced production problems at another plant in Allston, Mass. That plant is running again but will focus on production of another product.

Fabry disease is a genetic disorder that causes accumulation of a certain type of fat in the body's cells, the AP reported.

Severe Allergic Reactions After Vaccinations Rare: Study

Sudden, serious allergic reactions are "extremely rare" after childhood vaccinations, according to a new study.

British researchers reviewed 15 suspected cases of children younger than 16 who experienced anaphylaxis after receiving vaccinations to protect against measles, HPV, meningitis, hepatitis or typhoid, CBC News reported.

Only seven of the cases were confirmed as anaphylaxis. Six children required an injection of adrenaline and intravenous fluids and one child did not require treatment. All seven children recovered fully.

Based on their findings, the researchers calculated that the incidence of anaphylaxis is 12 cases for every 100,000 vaccine doses, CBC News reported.

The study was published online Monday in the Archives of Disease in Childhood.

Rep. Giffords to Resign From Congress

Representaive Gabrielle Giffords announced Sunday that she will resign from Congress to focus on her recovery from injuries suffered in an assassination attempt last year.

The Democrat from Arizona wanted to return to Congress and was expected to have an easy re-election if she ran this year, but she decided in recent days that she could not continue her recovery and still serve as a member of Congress, The New York Times reported.

Giffords will resign after attending President Barack Obama's State of the Union address to Congress Tuesday. A special election will be held to determine who will fill the remainder of her term, which ends in November.

Giffords was shot in the head on Jan. 8, 2011, as she held a constituent meeting outside a grocery store in her hometown of Tucson. Six people died in the shooting.

In a video released online Sunday, Giffords told her constituents: "I don't remember much from that horrible day, but I will never forget the trust you placed in me to be your voice. Thank you for your prayers and for giving me time to recover. I have more work to do on my recovery, so to do what is best for Arizona, I will step down this week."

However, she also hinted at a possible future political comeback. "I will return, and we will work together for Arizona and this great country," Giffords said.

In a statement released Sunday, President Obama said: "Gabby Giffords embodies the very best of what public service should be. She's universally admired for qualities that transcend party or ideology -- a dedication to fairness, a willingness to listen to different ideas, and a tireless commitment to the work of perfecting our union."

FDA OK for Marijuana-Based Cancer Pain Drug Sought

A British company hopes to gain U.S. Food and Drug Administration approval for a marijuana-based mouth spray to treat cancer pain.

The product -- called Sativex -- contains two components of marijuana, delta9-THC and cannabidiol. The product has been approved in Canada, New Zealand and a number of European countries to relieve muscle spasms associated with multiple sclerosis, the Associated Press reported.

Sativex is currently in advanced clinical trials and maker GW Pharma hopes to get FDA approval for use of the medicine in cancer patients by the end of 2013.

Along with developing new applications for Sativex, GW Phamra is working on other drugs with different cannabis formulations, the AP reported.

Rwanda: Injectable Contraceptives May Increase Risk of Contracting HIV

She describes herself as a careful person, one who does not take chances with life. These, she says, are two principles that she thought could protect her from contracting HIV from her partner who is positive.

"We did all what our doctor told us to do, to keep me safe, we followed every order to the letter, but one day when my doctor told me I was no longer HIV negative, I felt as if life had come to an end, I thought I didn't get him right at first, but another test confirmed I was HIV positive," Jennifer Wanji said.

She says she demanded for a repeat of the test twice, but this did not change the situation. This only confirmed her fears. Her husband was also astounded by this news and sought to find out just what could have gone wrong. "I had always taken careful steps to protect my wife from contracting HIV, we have always gone for routine checkups and adhered to the doctors instructions on how to go about protecting ourselves from transmission. The news hit us hard," he says.

It is now two months since the couple received news of what could have possibly caused the transmission. Their doctor told them about a new study, which reveals that women who are HIV negative and using injectable contraception might face a two-fold risk of acquiring HIV from their infected partners. Wanjie had started using injectable contraception not more than six months before and for this the doctor told them that this could have triggered the infection.

Findings by Lancet Journal, a study in seven Sub-Saharan African countries potentially present an alarming quandary for women in Africa. It shows that women using hormonal contraception became infected at a rate of 6.61 per 100 person-years, compared with 3.78 for those not using that method.

The birth control method which is currently used by up to 6 percent of the 12 million women ages 15 to 45 on contraception in the East and Southern African region is preferred by women because it is just a shot every three months and birth control is taken care of. Although it is widely used, the injectable contraception is also associated with several other side effects including delayed return to fertility after using it, menstrual irregularities, weakness and fatigue, abdominal discomfort, hair loss and nervousness.

The study conducted from 2004 to 2010 involved 3,800 couples in several African countries including Kenya, Tanzania and Uganda. Researchers followed most couples for two years, had them report their contraception methods, and tracked whether the uninfected partner contracted HIV.

However, the International Planned Parenthood Federation (IPPF) considers the findings as not conclusive enough and therefore should not be used to draw conclusion on hormonal contraceptive use and its potential role in increasing users risk of HIV acquisition. "There is need for additional research for confirmation and better understanding of the study's findings and addressing of key outstanding issues and questions," the agency's African regional office says.

Voicing similar concerns are family planning experts who have advised women to continue using the injectable contraceptive -- Depo Provera --amid alarms that it could double the risk of contracting HIV. Dr Alex Mbaya told Xinhua that the report is not conclusive enough to advise women to stop using the method. "Women should keep using the injectables, in my own opinion, enough research was not done to come up with such conclusions," he says.

He says the research was not originally set out to find a linkage between contraceptive use and HIV but this conclusion was a secondary finding of another study that was being carried out. "We agree what happened to Wanjie could have been as a result of using the method but the report does not show how the injections increase the risk of contracting HIV, the facts are not very clear, women using the method should be assured, no cause for alarm," he says.

10 Post-Workout Partner Challenges

Sure, your workout session's over, but that doesn't mean you don't have enough left in you to challenge your workout partner to a little friendly competition. Throwing a little pride into the mix is a great way to kick your motivation up into the next gear. We talked to Ben Musholt, a physical therapist in Portland, Oregon for over 10 years and the founder of Strength Mob and BPM Rx, Inc. about some easy ways to challenge your gym buddy and yourself.

When you think you're running on empty, test yourself and your friend, and you'll be surprised how much strength you dig up.

1. The Push-up Game:

The first person begins with 10 push-ups. The second person matches the 10, and then does as many extra as he can. The next person does the difference between the two sets, and adds on another few reps. You must maintain a push-up position for the duration of the game, without letting your knees touch the ground. The game cycles continuously and is won by whoever completes all of the push-ups and doesn’t collapse.

2. Squat Hold:

Square off from your partner and get into a deep squat position, with hips and knees below 90 degrees of flexion, however don’t drop all the way to your heels. The guy who can maintain the squat for the longest time, without standing up or dropping backward, wins the challenge.

3. Single Leg Bench Get-up:

Sit side-by-side on a weight bench, and keep one leg elevated off the floor. Start standing up together, on one leg, keeping pace at an easy cadence. The last person to be able to complete the get-up on one leg wins.

4. Pull-up Add-on:

The challenge begins with someone doing a single pull-up on a high bar. The next person completes a pull-up, then adds a second pull-up or another hanging movement. Mix it up with a clapping pull-ups, circular pull-ups, or hanging leg raises. The game builds to a progressively longer series of hanging motions and ends when someone can’t match the last movement.

5. Handstand Hold:

Kick up to a handstand against a wall, facing away from the wall. Lock out your arms and settle in for the long haul. Whoever can stay up the longest wins the challenge.

6. Towel Drag:

Find some open floor space, like an indoor basketball court or long hallway. Get in a plank position and place an old towel under your feet. Drag yourself in a plank position, pulling with your arms, as far as you can. The person that completes the most laps is the winner.

7. Side Plankers:

Pick a side and then get in position. Hold a side plank with your top arm and leg elevated, for as long as you can. Longest duration wins.

8. Wall walks:

Start in a plank position, with your feet on the floor, near a wall. Walk your hands towards the wall and feet upward, to finish with your chest against the wall in a handstand position. Crawl back to the starting position. Most reps wins.

9. V-sit:

Sit on the ground and lift your legs up, so that you are balanced in a “V” position, with arms elevated too. Hold for as long as you can. Winner stays up the longest.

10. Wall Sit with Plate Raise:

Grab a 10-pound weight plate, then find some empty wall space. Settle into a wall sit with your hips and knees bent to 90 degrees. Raise the weight plate above your head, with your arms straight. Sit in this position until you either have to stand or lower the weight. Last man in position is the champ.

Hip Fracture Patients Often Have Other Health Problems

MONDAY, Jan. 23 (HealthDay News) -- Weight loss and malnutrition are among the medical conditions that increase treatment costs and the length of hospital stays for older adults with hip fractures, a new study finds.

More than 250,000 hip fractures occur each year in the United States, often resulting in hospitalization, surgery, extended periods of rehabilitation and/or long-term disability, and admission to a nursing home.

This study looked at coexisting medical conditions (comorbidities) that affect treatment costs and the length of hospitalization for hip fracture patients. The researchers examined 2007 hospital discharge data from 32,440 patients treated at more than 1,000 hospitals in 40 states. Nearly 80 percent of the patients were 75 or older and 72 percent were women.

Most of the patients had two or three comorbidities. Only about 5 percent had no other health conditions. High blood pressure affected 67 percent of the patients and was by far the most common comorbid condition.

Other comorbidities included: deficiency anemias (disorders caused by a lack of certain nutrients, such as iron or vitamin B12); fluid and electrolyte disorders; chronic lung diseases; diabetes; neurological disorders; hypothyroidism; and congestive heart failure.

The researchers found that comorbidities significantly increase treatment cost and length of hospital stay. Hip fracture patients who were very thin or malnourished had the greatest increased costs, following by those with pulmonary circulatory disorders that affect blood flow to and from the lungs.

Recent weight loss or malnutrition also had the greatest impact on hospitalization, increasing the length of hospital stay by 2.5 days. Hospital stays were about a day longer for patients with congestive heart failure or pulmonary circulation disorders.

Other comorbidities that lengthened hospital stay were fluid and electrolyte disorders, paralysis, and conditions contributing to blood clots.

The study was published in the Journal of Bone and Joint Surgery.

Two major issues require further investigation, according to Dr. Kevin Black, one of the study authors and professor and chair of orthopedics and rehabilitation at Penn State College of Medicine.

"First, we need to better understand the total cost of caring for hip-fracture patients. Our study focused only on acute hospitalization, but care typically extends well beyond this, since many patients are discharged to rehabilitation and skilled-nursing facilities," Black said in a journal news release.

"Second, this study did not investigate the quality or outcomes of care. As our population ages, there is reason to believe that the number of hip fractures will increase. Having a better understanding of the comorbidities that affect hip-fracture patients hopefully will lead to the development of strategies to more effectively care for these patients."

Dyslexia's Brain Changes May Occur Before Kids Learn to Read

MONDAY, Jan. 23 (HealthDay News) -- New imaging research shows that the reduced brain activity associated with the onset of dyslexia appears to develop before, not after, a child starts to read.

Key parts of the brain's rear left hemisphere critical to language processing do not undergo activity changes as a consequence of dyslexia, the study suggests, but may instead be part of the cause.

The finding could ultimately help clinicians screen for at-risk children at an early pre-reading age, when interventions to reduce the severity of the condition might be most effective.

"We already knew that children and adults with a diagnosis of dyslexia show brain alterations within the left posterior -- back -- part of the brain," said study co-author Nadine Gaab, an assistant professor of pediatrics in the neuroscience program at Harvard Medical School and Children's Hospital Boston. "However, it was unclear whether these alterations are a result of dyslexia [that] show up after years of reading failure or whether they predate the reading onset," she noted.

"[Here] we could show that they predate reading onset," Gaab said. "This suggests that children are either born with it or that it develops within the first few years of life."

The study, published in the Jan. 23 issue of the Proceedings of the National Academy of Sciences, focused on 36 healthy kindergarteners aged 5 and 6 years who had not begun to read.

Half of the children were at a high risk for developing dyslexia, as at least one of their immediate family members had been previously diagnosed with the disability. None of the children had difficulty with hearing or vision, and none had a history of either neurological or psychological illness.

After completing standard pre-reading language and vocabulary skills assessments, all of the children participated in a couple of audio-identification tasks. First, they were asked to listen to a male or female voice uttering a single word twice and then indicate if the two words sounded the same. Next, they listened to a pairing of words and were asked to indicate if the gender of the voice uttering each successive word was the same.

Throughout the testing, the children also underwent functional MRI (fMRI) to monitor their brain activity, with particular focus on two regions of the rear, left brain: the bilateral occipitotemporal and left temporoparietal areas. Both have previously been shown to have a role in dyslexia.

The results: Children in the at-risk group were found to have reduced brain activity in the two key brain areas, compared to their peers with similar age and IQ who did not have family risk factors.

In addition, the research team found that among at-risk pre-reading children there was no evidence of activity increases in key frontal lobe brain regions previously linked to dyslexia. This, they said, suggested that the brain's method for trying to compensate for the problems associated with dyslexia does not appear to be set in motion until after children begin to read.

"Early identification of children at risk in kindergarten or even before then offers a chance to reduce the clinical, psychological and social implications of reading disability/dyslexia," Gaab said. "Identifying early predictors will also help educators, parents and scientists to find ways to support the academic and cognitive development of children with reading disability/dyslexia and may also lead to strategies that will reduce the severity of reading disability."

Guinevere Eden, director of the Center for the Study of Learning and a professor of pediatrics at Georgetown University in Washington, D.C., expressed enthusiasm for what she deemed to be "the first study of its kind."

"The question has always been, are these physiological changes the result of dyslexia or are they there to begin with?" said Eden, who is also the immediate past-president of the International Dyslexia Association.

"And so what's interesting about this study, is that by using non-invasive tools, they were able to find that the kind of differences that have been shown in older people with dyslexia are apparently already present in children at risk for dyslexia before they even begin to read," Eden said.

"And that means they have found a physiological signature for a child who is likely at risk for dyslexia, which will be of great help in doing what everyone really wants to do: identifying and treating children with dyslexia as early as possible," Eden added.
