Background: The oral administration of vitamin B12 offers a potentially simpler and cheaper alternative to parenteral administration, but its effectiveness has not been definitively demonstrated. The following protocol was designed to compare the effectiveness of orally and intramuscularly administered vitamin B12 in the treatment of patients [greater than or equal to]65 years of age with vitamin B12 deficiency. Methods: The proposed study involves a controlled, randomised, multicentre, parallel, non-inferiority clinical trial lasting one year, involving 23 primary healthcare centres in the Madrid region (Spain), and patients [greater than or equal to]65 years of age. The minimum number of patients required for the study was calculated as 320 (160 in each arm). Bearing in mind an estimated 8-10% prevalence of vitamin B12 deficiency among the population of this age group, an initial sample of 3556 patients will need to be recruited.Eligible patients will be randomly assigned to one of the two treatment arms. In the intramuscular treatment arm, vitamin B12 will be administered as follows: 1 mg on alternate days in weeks 1 and 2, 1 mg/week in weeks 3-8, and 1 mg/month in weeks 9-52. In the oral arm, the vitamin will be administered as: 1 mg/day in weeks 1-8 and 1 mg/week in weeks 9-52. The main outcome variable to be monitored in both treatment arms is the normalisation of the serum vitamin B12 concentration at weeks 8, 26 and 52; the secondary outcome variables include the serum concentration of vitamin B12 (in pg/ml), adherence to treatment, quality of life (EuroQoL-5D questionnaire), patient satisfaction and patient preferences. All statistical tests will be performed with intention to treat and per protocol. Logistic regression with random effects will be used to adjust for prognostic factors. Confounding factors or factors that might alter the effect recorded will be taken into account in analyses.DiscussionThe results of this study should help establish, taking quality of life into account, whether the oral administration of vitamin B12 is an effective alternative to its intramuscular administration. If this administration route is effective, it should provide a cheaper means of treating vitamin B12 deficiency while inducing fewer adverse effects. About 1 in 600 babies in the United States is born with a cleft lip and/or cleft palate, according to the Cleft Palate Foundation. The defect can range from a small notch in the lip to a grove that runs into the roof of the mouth. It can occur in isolation or in combination with other craniofacial birth defects. (A craniofacial disorder refers to an abnormality of the face and/or head.)

The first step in managing craniofacial patients is ensuring a safe airway. There's also a great potential for nasal obstruction and sleep apnea. And patients are at increased risk of developing upper airway problems such as sinusitis, laryngitis and rhinitis.

Hearing loss is common and often progressive. Thus, in addition to receiving standard newborn hearing screening, craniofacial patients should continue to receive periodic hearing tests, Rosenthal and colleagues write.

Craniofacial patients typically require several corrective surgeries, performed in staged fashion. Surgeons and anesthesiologists should be aware of the potential challenges these patients may have with general anesthesia.

The authors recommend a multidisciplinary approach, beginning with genetic counseling to determine the cause of the malformation, to inform parents about what to expect and to learn about the implications for other family members.

In addition to otolaryngologists, other specialists who typically care for craniofacial patients include pulmonologists, gastroenterologists, dentists and orthodontists. Depending on the congenital condition, a patient also may see pediatric specialists, such as cardiologists, ophthalmologists, neurosurgeons, endocrinologists, urologists, nephrologists and orthopaedic surgeons.

Most patients also need additional support services, including case management (social work), psychology or psychiatry, speech pathology, physical therapy, occupational therapy and other educational services.

The authors detail the management of major conditions associated with craniofacial disorders, including Apert syndrome, Crouzon syndrome, CHARGE syndrome, chromosome-22 and 22q11.2 deletion syndromes, DiGeorge syndrome, velocardiofacial syndrome (VCFS), Opitz G/BBB syndrome, congenital nasal pyriform aperture stenosis, Moebius syndrome, mandibulofacial dysostosis syndromes, oculoauricular-vertebral spectrum (OAVS), Goldenhar syndrome and Stickler syndrome.

Rosenthal is an assistant professor in the Departments of Otolaryngology and Pediatrics of Loyola University Chicago Stritch School of Medicine. Her special interests include cleft lip and palate, breathing difficulties, facial lesions, nasal/sinus problems, neck masses, pediatric ear, nose and throat, special-needs children and speech delay.

The associations between EL and the risk of obesity, hypertension, diabetes, myocardial infarction, stroke and anxiety disorders or depression, nutrition, exercise, and smoking were evaluated. University education served as the reference category (EL4), the lowest educational level was required schooling only (EL0). Results: Only among women did the risk for diabetes mellitus and hypertension increase with decreasing educational level with the highest rates for EL0 OR [95% CI] adjusted for age, income, family status and lifestyle: 3.7 [1.7-8.0], and 2.5 [1.8-3.5], respectively. Only among the men, however, did the risk for stroke increase with decreasing educational level adjusted OR for EL0: 8.5 [1.7-42.7]. For anthropometric measures and lifestyle factors in both sexes the risk increased with decreasing EL. Conclusion: EL affects lifestyle, overweight and obesity in both sexes. The apparent sex-specific differences in the association between the prevalence of some chronic disease with EL call for further investigation.

Breast cancer is the most common cause of cancer-related deaths in women worldwide. An estimated 40,000 women in the United States are expected to die from the disease this year. In the developed world, approximately two-thirds of all breast cancer cases are ER+. Highly dependent on their estrogen receptors for growth and survival, these tumor cells are sensitive to hormonal therapies, making drugs like tamoxifen and fulvestrant effective first-line therapies for many patients.

However, in patients with breast cancers that recur or metastasize after initial hormonal therapy, long-term remission is uncommon and life expectancy is about three years. Currently, these patients have few therapeutic options. By adding an HSP90 inhibitor, Lin and Whitesell hope to hobble the resistant cancer cells, making them less likely to escape the effects of fulvestrant.

Lin likens a malignant tumor to a car on a highway in a high-speed chase. As the car hurtles down the road, police may erect a roadblock to stop it. In the case of ER+ breast cancer, that roadblock is hormonal therapy. Unfortunately, a tumor may mutate enough to find a detour around the roadblock and continue on its destructive course. Adding a heat shock protein inhibitor to the mix may thwart the cancer's ability to find an alternate route.

"Because of the way HSP90 inhibitors work, it's like throwing a bunch of roadblocks across a bunch of detours," says Lin, who will be running the clinical part of the trial. "So the idea is to try to block these alternative pathways that might allow the tumor to grow."

"Inhibiting HSP90 in tumors may not cause them to roll over and die," says Whitesell, who with support from Susan G. Komen for the Cure will be analyzing blood and tissue samples from trial participants. "But it may change the biology of the cancer to make it more susceptible to other things that we already know work against the cancer, like fulvestrant."

The trial's premise is based on years of work the Lindquist lab has conducted on heat shock proteins. That experience is invaluable, according to Sandro Santagata, a pathologist in the Lindquist lab who will also be examining patient tissue samples.

"The Lindquist lab has a great basic scientific understanding of the biology of the heat shock system," says Santagata. "That foundation will hopefully provide important insights to increase the likelihood of success for this trial and if it doesn't work, an understanding of why. That's always a tricky part. Figuring out the 'why' is always complicated so it helps to have people like Susan and Luke who have a lot of background in the system you're studying."

For her part, Lindquist says she is looking forward to testing the therapeutic potential of targeting HSP90.

"I am very excited about the translational applications of some of the basic science we have been working on over the years," says Lindquist, who is also a professor of biology at MIT and a Howard Hughes Medical Institute (HHMI) Investigator. "Luke and I are thrilled to be working with Nancy Lin."

He said using the drug in sero-discordant couples--where one is HIV-positive and the other negative--can cut chances of transmission by as much as 96% in healthy people.

Transmission risk

Using other means to cut HIV transmission is crucial on account of behaviour.

THURSDAY, May 24 (HealthDay News) -- Preeclampsia, a dangerous spike in a woman's blood pressure during pregnancy, may predispose offspring to high blood pressure in childhood and young adulthood, a new study finds.

From early in life, these children have distinct cardiovascular risk factors that may put them at risk for health problems later on, the British researchers said.

"A pregnancy complicated by preeclampsia is an early warning sign that both the mother and offspring are going to be at greater risk of developing high blood pressure later in life," said lead researcher Dr. Paul Leeson, from the department of cardiovascular medicine at the University of Oxford in England.

The findings suggest a need to monitor these children, the authors said. "There is likely to be value in considering a history of preeclampsia to understand better a person's risk of developing high blood pressure," Leeson said.

High blood pressure, also called hypertension, can lead to stroke, heart attack and kidney failure.

Research into the biology underlying this association may also help identify new ways to prevent high blood pressure, Leeson added.

The report, published May 21 online, appears in the June print issue of Pediatrics.

For the study, Leeson's team reviewed 18 studies published between 1948 and 2011 that dealt with cardiovascular risk factors of children and young adults exposed to preeclampsia and those not exposed.

This type of study, which involved more than 45,000 individuals, is called a meta-analysis and is designed to look for common patterns in unrelated studies.

In their analysis, the researchers found that kids exposed to preeclampsia had higher blood pressure readings in childhood and as young adults, compared with those who were not exposed.

Systolic blood pressure of those exposed to preeclampsia was 2.39 mm Hg higher on average than that of those whose moms had healthy pregnancies, and diastolic pressure was 1.35 mm Hg higher on average, the researchers noted. In a blood pressure reading of 120/80, the first number -- 120 -- is the systolic pressure.

Over time, this difference in systolic blood pressure would increase a person's risk of death from heart disease by about 8 percent and stroke by 12 percent, the authors said.

Children and young adults exposed to preeclampsia also had a higher body mass index (BMI) than children not exposed, the researchers say. BMI, a calculation of body size based on height and weight, is another risk factor for cardiovascular disease.

The study does not prove that preeclampsia causes cardiovascular disease, however. It merely shows an association between the two.

Dr. Natalie Meirowitz, chief of maternal-fetal medicine in the department of obstetrics and gynecology at Long Island Jewish Medical Center in New Hyde Park, N.Y., said this study points to a major public health issue.

"This study really says the intrauterine environment affects a child's vascular function as an adult," Meirowitz said.

"It may be obesity that is driving this," added Meirowitz. More pregnant women are obese, which is a risk factor for preeclampsia.

"We really need to consider cardiovascular disease that comes from the intrauterine environment and understand it better so we can prevent future cardiovascular disease," she said.

Obesity in pregnancy is a modifiable risk factor, Meirowitz said. "There isn't enough attention paid to it and the effect it can have on children later in life," she said.

Africa: OIG Identifies Areas That Require More Attention

CCM oversight, sub-recipient management and the role of local fund agents all require greater attention, according to the Office of the Inspector General (OIG). These comments are included in the OIG's Progress Report for November 2011-March 2012, which was presented to the Global Fund Board at its meeting in May in Geneva. The OIG has offered to work with the Secretariat to address all three areas.

The OIG said that risks associated with governance and oversight at the country level, particularly through CCMs, were identified in the majority of audits the OIG has undertaken. The OIG also said that the LFA function requires "extensive reinvention" if LFAs are going to fulfil their role of ensuring that grant implementation is sound.

The OIG said that it has a number of audit reports in the pipeline, all of which include findings covering all grants in a given country. However, the OIG said, all audits and diagnostic reviews initiated after November 2011 will be restricted to grants starting from Round 6.

The OIG issued four reports at the end of April (three audits and one diagnostic review). The language of those reports, as well as this progress report, was more subdued than the language of previous reports from the OIG. In the progress report, the OIG said that it is paying greater attention to "the tone and style" of its communications. The OIG said that it expects to issue another five or six reports on audits and diagnostic reviews "several weeks" after the Board meeting. It added that "a number" of investigation reports will be issued in the second and third quarters of 2012.

The OIG said that, in future, reports on internal reviews (i.e., audits of programmes at the Secretariat) will be shared with the General Manager, and will be summarised for the Audit and Ethics Committee, but will no longer be posted on the Global Fund's website.

In its report, the OIG said that its Investigation Unit had planned to return to Mali to continue its investigation of Global Fund grants to that country, but that the plans were interrupted by the recent coup. The OIG said that its team will return when the domestic situation settles and it is safe and productive to continue the in-country work.

In its report, the OIG said that "given the evolution of the caseload," the focus of the Investigations Unit is slowly turning from Africa to Asia.

The OIG said that a co-operation agreement covering the OIG's investigatory activities has now been finalised with the United Nations Development Programme (UNDP). The OIG said that it recognises that its relationship with the UNDP "needs to be built on mutual trust." It added that it has shared information with the UNDP on certain investigative efforts, shared drafts of reports in countries with the UNDP for their input, exchanged information on the respective audit plans and schedules of the two entities, and benefitted from reading the UNDP's audit reports through a new remote access agreement. The OIG added that it is currently exploring options for having UNDP auditors join its team for audits and reviews of sub-recipients.

Children born with cleft lip, cleft palate and other craniofacial disorders face numerous medical challenges beyond appearance.

Patients can face serious airway, feeding, speech and hearing problems, as well as social and psychological challenges, Laura Swibel Rosenthal, MD, of Loyola University Medical Center and colleagues write in the June 2012 issue of Otolaryngologic Clinics of North America.

"The management of patients with craniofacial syndromes is complex," Rosenthal and colleagues write. "Otolaryngologic [ear-nose-throat] evaluation is of paramount importance in providing adequate care for this patient population."

About 1 in 600 babies in the United States is born with a cleft lip and/or cleft palate, according to the Cleft Palate Foundation. The defect can range from a small notch in the lip to a grove that runs into the roof of the mouth. It can occur in isolation or in combination with other craniofacial birth defects. (A craniofacial disorder refers to an abnormality of the face and/or head.)

The first step in managing craniofacial patients is ensuring a safe airway. There's also a great potential for nasal obstruction and sleep apnea. And patients are at increased risk of developing upper airway problems such as sinusitis, laryngitis and rhinitis.

Hearing loss is common and often progressive. Thus, in addition to receiving standard newborn hearing screening, craniofacial patients should continue to receive periodic hearing tests, Rosenthal and colleagues write.

Craniofacial patients typically require several corrective surgeries, performed in staged fashion. Surgeons and anesthesiologists should be aware of the potential challenges these patients may have with general anesthesia.

The authors recommend a multidisciplinary approach, beginning with genetic counseling to determine the cause of the malformation, to inform parents about what to expect and to learn about the implications for other family members.

In addition to otolaryngologists, other specialists who typically care for craniofacial patients include pulmonologists, gastroenterologists, dentists and orthodontists. Depending on the congenital condition, a patient also may see pediatric specialists, such as cardiologists, ophthalmologists, neurosurgeons, endocrinologists, urologists, nephrologists and orthopaedic surgeons.

Most patients also need additional support services, including case management (social work), psychology or psychiatry, speech pathology, physical therapy, occupational therapy and other educational services.

The authors detail the management of major conditions associated with craniofacial disorders, including Apert syndrome, Crouzon syndrome, CHARGE syndrome, chromosome-22 and 22q11.2 deletion syndromes, DiGeorge syndrome, velocardiofacial syndrome (VCFS), Opitz G/BBB syndrome, congenital nasal pyriform aperture stenosis, Moebius syndrome, mandibulofacial dysostosis syndromes, oculoauricular-vertebral spectrum (OAVS), Goldenhar syndrome and Stickler syndrome.

Rosenthal is an assistant professor in the Departments of Otolaryngology and Pediatrics of Loyola University Chicago Stritch School of Medicine. Her special interests include cleft lip and palate, breathing difficulties, facial lesions, nasal/sinus problems, neck masses, pediatric ear, nose and throat, special-needs children and speech delay.

Irritable Bowel Linked To Gut Bacteria, Definitively

A new study of Greek patients shows that overgrowth of bacteria in the gut is definitively linked to irritable bowel syndrome (IBS). It is the first to use the "gold standard" method of examining gut bacterial cultures to connect bacteria to the cause of a disease that affects some 30 million Americans. The researchers say their findings confirm antibiotics are a successful treatment for IBS.

Previous studies have suggested a link between gut bacteria and IBS, but they have been based on testing methane (a byproduct of bacterial fermentation) in the breath.

The findings, published in the May issue of Digestive Diseases and Sciences, corroborate those of previous clinical trials at Cedars-Sinai that showed antibiotics are effective against IBS.

Study author Mark Pimentel is director of the Cedars-Sinai GI Motility Program. He commented in a statement to the press released on Friday that:

"While we found compelling evidence in the past that bacterial overgrowth is a contributing cause of IBS, making this link through bacterial cultures is the gold standard of diagnosis."

"This clear evidence of the role bacteria play in the disease underscores our clinical trial findings, which show that antibiotics are a successful treatment for IBS," he added.

For the study, Pimentel and colleagues from Sismanogleion General Hospital in Athens, Greece, and from the University of Athens, examined samples of small bowel cultures from over 320 Greek patients to confirm the presence of small intestinal bacterial overgrowth (SIBO). The patients were all scheduled to receive upper gastrointestinal (GI) tract endoscopy.

They found that more than a third of the patients with IBS had SIBO, compared with only 10% of those without IBS.

Of patients with diarrhea-predominant form of IBS, 60% had SIBO, compared to just over 27% without the diarrhea form.

The researchers used the Rome II criteria to define IBS. The Rome criteria is a system, based on clinical symptoms, of classifying disorders of the digestive system in which symptoms can't be explained through presence of tissue abnormality. As well as IBS, other disorders that are defined using Rome criteria include dyspepsia, functional constipation, and functional heartburn.

Pimentel and colleagues conclude:

"Using culture of the small bowel, SIBO by aerobe bacteria is independently linked with IBS. These results reinforce results of clinical trials evidencing a therapeutic role of non-absorbable antibiotics for the management of IBS symptoms."

IBS is the most common gastrointestinal disorder in the US. Symptoms include painful bloating, constipation, diarrhea or an alternating pattern of both.

Many people with IBS avoid social interactions because they are embarassed by their symptoms.

Ten years ago Pimentel went against the thinking of the time when he proposed bacteria played a key role in IBS. Since then he has led clinical trials that have shown rifaximin, a targeted antibiotic absorbed only in the gut, is an effective treatment for IBS.

Pimentel said in the past, treatments have focused on alleviating symptoms. But "patients who take rifaximin experience relief of their symptoms even after they stop taking the medication".

"This new study confirms what our findings with the antibiotic and our previous studies always led us to believe: bacteria are key contributors to the cause of IBS," he affirmed.

However, they also found that some staff members were reluctant to test patients because of how much time it would take to either care for a newly diagnosed patient or link them to other care. That's disturbing, as is the fact that a number of clinics simply didn't think that HIV testing was important or part of their mandate of care.

While it's wonderful that HIV is becoming more treatable, it still puts an incredible burden on those who suffer from it as well as their communities. Regular testing could start to change that - by helping to identify the many HIV positive people who have no idea of their infection and getting them prompt and effective help. We just have to convince doctors that it's important, and possible, for them to get it done.

As I blogged about on Monday, it's been difficult getting primary care providers to start implementing universal testing for HIV. However, it should be easier to at least improve testing for high risk groups such as drug users... right?

Sort of. Although HIV rapid tests are available, and particularly well suited for use in high prevalence settings such as drug treatment centers, the resources needed to provide risk-reduction counseling along with testing can sometimes be prohibitive. That's why a recent paper by Dr. Lisa Metsch and colleagues in the American Journal of Public Health was so interesting. They found that when risk-reduction counseling was provided along with HIV testing in a drug counseling center, it didn't have any significant benefits for preventing unprotected vaginal or anal intercourse.

While that's slightly depressing, there is one upside to such a result. It means that drug treatment centers could implement onsite rapid testing without needing to worry about whether they have the capacity to combine it with HIV-counseling. That's a very good thing. The study also found that onsite testing was a far more efficient way of helping drug users find out their infection status than sending them away to get tested somewhere else.

Question of the Week: Alcohol and Fitness

The science behind fitness and health is wild, crazy and ever changing. One minute a study supports a particular claim, then next it's the worst thing you could humanly do to or for yourself. Sometimes you'll even find the same questions looming around the industry with mixed reviews, perspectives and findings. In efforts to calm the madness, each week, here at MensFitness.com we'll scour the internet, tap into forums and ask our friends on Facebook and Twitter about what question in fitness we can get some firm answers to.

This week, we take a look at alcohol and how it fits into a "fitness lifestyle".

 

Q: Why is the fitness industry as a whole so "anti-alcohol"? Isn't there any room for a drink or two at major event? What kind impact does that really have?

 

A: The fitness industry’s anti-alcohol stance is due, in large part, to the (lack of) nutritional content contained in alcoholic beverages. Ethanol, the type of alcohol found in drinks, has toxic metabolic byproducts called acetaldehyde and acetate. Both by-products help create that queasy nauseous feeling you get when you’ve put a few too many back. Chronic ingestion of alcohol can also mess with your digestion, making it difficult for your body to absorb nutrients like amino acids, B vitamins, and impairs protein synthesis. One study also suggests that two to three beers per day can lower one’s testosterone levels.

Alcohol consumption can also mess with a carefully planned out diet. Many alcoholic drinks are calorically very dense and packed with sugar. If you’re mixing your alcohol with things like energy drinks or juices from concentrate, or even having a few glasses of wine, you are giving yourself a sugary rush with each sip. Let’s also not forget that alcohol totally impairs your judgment. Those late night post-bar pizza runs can’t be too good for your summer beach body.

However, if users are able to practice some moderation with alcohol, a drink or two on occasion will not effectively poison your body and make you irreparably fat. There are some studies that suggest moderate alcohol consumption can provide health benefits. Much has been made of the heart healthy components of a glass of red wine, but did you know that moderate beer consumption can improve your immune response and can reduce harmful C-reactive proteins (linked to heart disease)? All in all, alcohol is harmful when overdone. It can mess with your digestive system, liver, and diet. But if done within moderation and with clear judgment, it can be beneficial, and of course a ton of fun.
