THURSDAY, March 22 (HealthDay News) -- Breast cancer is more likely to recur in women over 50 who have denser breast tissue, researchers report, noting that doctors should take breast density into account when making initial treatment decisions.
The Swedish study found that women with denser breasts had nearly twice the risk of recurrence, either in the same breast or in the surrounding lymph nodes, than women with less dense breasts.
"We found that if you have a PD [percentage density] at diagnosis of 25 percent or more, you have an almost twofold increased risk of local recurrence in the breast and surrounding lymph nodes than women with a PD of less than 25 percent," said Dr. Louise Eriksson and colleagues from the Karolinska Institute in Stockholm.
"However, density does not increase the risk of distant metastasis [spread] and has no effect on survival," Eriksson said.
Breast density, which can be determined through mammography, varies among women and decreases with age, especially at menopause, the researchers noted.
For the study, the researchers reviewed mammograms and treatment results for almost 1,800 postmenopausal women, aged 50 to 74, who were part of a larger study of women diagnosed with breast cancer in Sweden between 1993 and 1995.
The findings were presented Wednesday at the European Breast Cancer Conference, in Vienna.
It is known that dense breast tissue increases the odds of developing cancer, but whether breast density might influence recurrence has not been established.
"Our study shows that breast density before or at diagnosis should be taken into account even after diagnosis, for instance, when deciding on adjuvant [secondary] treatment and follow-up routines," Eriksson said in a meeting news release. "Perhaps women with dense breasts should be followed more frequently or for a longer period of time in order to quickly spot any local recurrence."
The research showed that breast density differs greatly. "In the group of women I studied, those with the lowest percentage density had breasts that were less than 1 percent dense, whereas those with highest PD had 75 to 80 percent dense breasts," Eriksson said. The average was 18 percent, she noted.
She said the findings are significant because of the detailed information on each woman and the overall size of the study. "This is one of the largest studies to date studying mammographic density, tumor characteristics and prognosis," Eriksson said in the news release.
Why dense breasts influence cancer is still unclear, and researchers say more work is needed before the findings could influence medical practice.
David Cameron, a professor at the University of Edinburgh in Scotland, said in the news release that the study is more thought-provoking than practice-changing, "since it is not clear what a patient, or her physician, should do if the mammogram shows a higher density of the normal breast tissue."
Research presented at meetings is considered preliminary until published in a peer-reviewed medical journal.

Zelboraf (vemurafenib), manufactured by Roche, has been approved by the European Commission, for treating patients with BRAF V600 mutation-positive metastatic melanoma, a deadly, and dangerous type of skin cancer. Zelboraf works by seeking out the mutated parts of the BRAF protein, found in about 50% of all melanoma cases, and blocking its action.
Hal Barron, M.D., head of Global Product Development and Chief Medical Officer at Roche said:
"Today's approval is important news for people with BRAF mutation-positive metastatic melanoma as Zelborad significantly improves patient survival and exemplifies the benefits that Roche's personalized approach to medicine can provide for patients, physicians, and society."
The Roche cobs 4800 BRAF V600 Mutation Test, a diagnostic test co-designed by Roche, to figure out which patients were eligible for treatment, showed that Zelboraf is the only known treatment which benefits the survival rates in some patients with accelerated melanoma who possess the BRAF V600 mutation, and who have been treated before or have not previously been treated.
Zelboraf tests showed many improvements, compared with chemotherapy. The risk of mortality was lowered by 63% in patients who received Zelboraf. It greatly improved life expectancy, compared to normal treatment by an overall survival rate of 13.2 months. Chemotherapy treatment usually gives patients a 9.6 month window of life expectancy after treatment.
Zelboraf became the only and the first FDA (Food and Drug Administration) approved medicine for improving survival rates among people with the BRAF V600 mutation in 2011. It was approved at the same time as the cobas 4800 BRAF V600 Mutation Test. Currently, they are both approved for use in the U.S., are CE-marked, and are commercially available in the EU.
Other countries which have recently approved the use of Zelboraf include: Israel, Brazil, Switzerland and New Zealand. Australia, India, and other countries around the world are reviewing the possibility of using Zelboraf. While other countries are reviewing Zelboraf and Roche awaits approval, a world-wide study for safety is giving Zelboraf to over 2000 people who have previously been treated, or not, for BRAF V600 mutation-positive metastatic melanoma.
All studies conducted regarding the safety of Zelboraf showed consistency.
Zelboraf, used for the monotherapy of adults with BRAF mutation positive unresectable or metastatic melanoma, is an oral-shaped, small molecule, kinase inhibitor. It is not recommended for melanoma patients who possess wild-type BRAF.
Melanoma, when it spreads to other parts of the body after the first diagnosis, is deadly. However, if found early, this type of cancer can be treated. A person with melanoma usually does not have a long life expectancy, sometimes only a few months. Normally, as little as 1 in 4 patients can be expected to live a year after being diagnosed.

It is now FDA approved in certain countries, and has been confirmed in the BRIM2 and BRIM3 tests to find tumors which carry the BRAF 4600E mutation. (These mutations happen in eight percent of all tumors)
It has been compared to Sanger sequencing, and has been proven recently to be better than this commonly used method. It is more sensitive and reliable than the Sanger sequencing in terms of finding mutations and gives quicker results.
BRIM2, a phase II study, enlisted 132 volunteers who had previously been treated for the V600E mutation-positive melanoma, and BRIM3, a phase III study, compared Zelboraf to dacarbazine chemotherapy in 675 patients who had previously not been treated for the V600E mutation-positive melanoma.
Zelboraf is being co-developed between Roche and Plexxikon(a Daiichi Sankyo Group member), with a 2006 license and collaboration contract.

Insects Behind Chagas Disease Are Feeding on Humans in the U.S.
TUESDAY, March 20 (HealthDay News) -- Insects that are part of a family of bugs that transmit Chagas disease are well-established and feeding on human blood in certain regions of the United States, a new study finds.
What remains a mystery, however, is why the insects infect millions in Central and South America with Chagas disease, a serious condition that can lead to life-threatening digestive and heart problems, yet few such cases have been documented in the United States.
So far, researchers have documented only seven cases of Chagas in the United States.
"The basic message is that the bug is out there, it's feeding on humans, and it carries the parasite, so there may be greater potential for humans to have the disease in the United States than previously thought," Lori Stevens, a biologist at the University of Vermont, said in a university news release. "Very likely, with climate change they will shift further north and the range of some species will extend."
In the study, which was published online March 14 in the journal Emerging Infectious Diseases, researchers collected specimens from the Reduviid family of insects in Arizona and California.
About 38 percent of the black, wingless insects, also known as "kissing bugs" because they bite sleeping humans around the mouth, contained human blood. Researchers said this was an unexpected finding because the 11 species of kissing bugs found in the United States were not known to feed on people.
More than half of the insects contained Trypanosoma cruzi, the parasite that causes Chagas disease.
Researchers speculate that the prevalence of the disease could be greater than previously thought.
"We think the actual transmission is higher than the seven cases we have identified," study co-author Patricia Dorn, an expert on Chagas disease at Loyola University in New Orleans, said in the news release. "But even with these findings, we think the transmission of Chagas -- of the T. cruzi parasite -- is still very low in the U.S."
The initial symptoms of Chagas disease can be vague, including fever and swelling around one eye and at the site of the bug bite. The disease can go into remission and reemerge years later as a more serious illness, resulting in potentially deadly digestive and heart problems.
The bug's saliva also can lead to severe allergic reactions.
Researchers theorized the bug hasn't become more widespread in the United States because most homes in the country are made with concrete basements, screened doors and windows, and tighter construction. In contrast, kissing bugs have thrived among the thatched roofs, stick-and-mud construction and dirt floors found in rural and poor regions of Central and South America.
Another theory is that unlike the kissing bugs found in other countries, the species found in the United States do not defecate while they are feeding on their host. As a result, there is less opportunity for the parasite that causes Chagas disease to enter the bloodstream.
One way to prevent the spread of Chagas disease is to use screens or close windows at night. Campers should close their tents or sleeping bags.
The study's authors said they hoped to examine a larger sample of kissing bugs from more areas in the United States.
"Chagas isn't going to spread fast, but it could spread," Stevens said. "Finding out how prevalent it is now would be a good idea."
